24/7 Kare4Me partners with home health care providers and in-home medical professionals to supplement their medical care with “non-medical” care, such as personal care, companionship, stand-by assistance, supervision, monitoring the client’s condition and reporting any changes to the medical professional.

 

24/7 Kare4Me´s role is not to replace the home health care nurse or medical professional but instead to help them provide the best care possible according to the care plan developed by the care team. Our Qualified Caregivers often act as the “eyes and ears” for the home health care medical professional, you and your family. For example, a home health care nurse or medical professional may schedule a daily or weekly visit; however, during the rest of the day or week, you or your loved one may require additional supervision, companionship and care.

 

24/7 Kare4Me becomes an integral part of the care team, which typically includes home a health care nurse, doctor, social worker/case manager and your family.
